WHO SHOULD APPLY? All parents are welcome to fill out an application in order to enroll their child. Leman Academy of Excellence is its own educational institution and therefore is not part of a school district, so there are no boundaries regulating scholar enrollment to attend Leman Academy. Therefore, scholars may attend Leman Academy of Excellence regardless of their address or location and we shall not limit admission based on ethnicity, national origin, gender, income level, disabling condition, or proficiency in the English language (per A.R.S. 15-184).
Leman Academy of Excellence does not discriminate on the basis of race, color, national origin, sex, disability, or age in its programs and activities, including in admission and enrollment. Leman Academy of Excellence abides by the procedures mandated by Section 504 of the Rehabilitation Act of 1973 and the Individuals with Disabilities Education Act (IDEA) and will provide eligible students with disabilities a free appropriate public education (FAPE), including following Section 504 accommodation plans and Individualized Education Programs (IEPs).

WHEN & HOW TO APPLY? Parents/guardians should complete the paperwork in the application package for EACH child they would like to enroll at Leman Academy. Once the online enrollment form has been completed and accepted, please download, fill out, and bring or mail-in ALL the documents in the pdf packet.

IMPORTANT: All forms must be filled out in full, signed, and submitted to the school by the child’s parent or legal guardian. The forms can be handed in at school or mailed/faxed to the following (health forms are to be handed in, mailed, or faxed as an added measure of safety to secure the privacy of personal health information per HIPAA). PLEASE NOTE: Parents may also be asked to provide custody information (e.g. court decision regarding sole custody), Medical Alert (e.g. physician’s instructions for severe health conditions), and Legal Alert (e.g. restraining order information) if applicable.
